years old or older who ARE NOT enrolled in a public school program.
The developmental screening is free of charge. However, a parent/legal guardian must give written permission for the screenings. An adult must accompany the child throughout the screening process.
It is recommended that you make an appointment by completing this form. Once the form is completed, our child find staff will contact you to complete the additional paperwork and to determine available dates and times.
You may contact Laurie Turner if you need further assistance with setting up an appointment at (843) 673-1161 or by email at lturner@fsd1.org.
